I think it's a bit unfair that Babcock and Wilcox should appear completely blameless, and my view is that the mention of possible sabotage here is outrageous. It's also a bit unfair to call the operators grossly incompetent when they trained on B&W simulators which could never reproduce the situation with the pressurizer.
"The Warning" by Gray and Rosen gives a wider spread of opinion.
