The bosu under the swiss ball is to reduce ground contact and make the balance point non-linear which again makes staying in perfect for that much harder. So if by "less efficient" you mean harder to control then having the bosu under the ball makes it harder to control or makes you less efficient at controlling it.

Are these less efficient if you don't have that "half ball" underneath the swiss ball? Or was he using that just for extra elevation?
