The worst thing you can do to embarrass yourselves in a very formal set-up is to make too much unnecessary movements on the table... but if I'm in a home of a friend, I think I would rather pick it up as a form of apology to the host.
I took etiquette classes ( mandatory in private school -4th grade) & they never once told us that if we were to drop a piece of silverware to LEAVE IT THERE!!! ..that is just gross & AWKWARD!! lol... who just leaves it there?? I wouldn't even be able to go on with the dinner cause i'd be so conscious of my fork on the floor with some food still on it and probably messing up the carpet/hardwood...

If you are at a large banquet or function, you should discreetly signal (Do not wave or flag like mad!) to a server to avoid that any staff serving could accidently slip on cutlery otherwise you risk having someone drop entire serving trays, plates of food, coffee onto a guest or staff - injuring someone & turning the entire affair into a disaster because of NEGLECT. It is YOUR responsibility to advise someone as soon as you can. But yes, do not get up to pick it if you are at a formal dinner.

This is the only thing I'm confused about.
If you don't pick it up, and ask the staff for a new fork/knife/napkin, won't it seem arrogant or stuck up or pretentious?
However, if you do pick it up, it is not classy to be putting silverware from the floor back onto the table.
It seems like a lose-lose situation...
