I finished the game with 70+ lives on my first play through, so it is pretty easy. I swapped it for Mario Kart 7 which has a lot more re-playability and a lot more of a challenge.

At the beginning of the level, before the midpoint, remember the music note box where the first star medal is? A few steps before the box is a blue grate. If you walk off the grate and fall down below into the sky below, there is a pipe. Inside are multiple coins and a fire flower.
