Most commonly pain is activated by nociceptors a form of receptor that has a high threshold to stimulus and therfor fires an action potential when a noxious amount of stimulus is present. TO break it down to you... pain is only a perception of the brain. Also the PAIN is not necesserily a binary on and off. The magnitude of stimulus is presented to the brain by the frequency of action potentials
