You unlock it after you get a gold metal on one of the levels. I think it was this one but I'm not sure.

What does the gold health do? I've never unlocked it and am curious. Does it mean extended health or is it just gold for fun?

Good multiplayer...if you haven't check out Nightfire...its 2x better then this game in most ways.
