I don't think you understand how marginal tax rates work. We have a graduated tax system. Tax rates increase on income over set levels, not to all income.

A marginal tax rate of 90% would only apply to income over say $10 million. A talented surgeon definitely can earn and deserve a few million dollars a year, and this would not be taxed at 90%. Only income OVER $10 million would be taxed at 90%.
