Look very closely at it's entire body when the head moves. It's entire body shifts in the direction of the head turning, not just her hands but her lower body as well. By this I would assume there is a pole attached to her head going down and ending at her feet (or below the dress). I want to say someone was turning her head from beneath her skirt with this pole (probably why we can't see the entire doll) but had some difficulty with a lot of friction when turning the pole.

This is real, I can't really explain it but my intuition tells me something is off. Usually when I watch these "scary" vids it's pretty obvious that they're fake.
