  • ATC are the police to ALL ground movement. Pan Am never should have reported "runway clear" just because ATC wanted to hear it. However, KLM was already rolling which in my view takes 100% of the blame since they were not given permission to take off from the ATC. Another interesting note, the pressure to remain in compliance with maximum hours allowed to fly per 24. There should be some exceptions when unexpected happenstance occures, say a 10% margin when schedules & mother nature clash.

  • @goodkarma33 He didn't report "runway clear." He said "OK will report runway clear", which is simply a readback of the instruction. Reading back an instruction is what you're supposed to do. But I agree it's poor communication to blame here. I personally think since it's not a hold short clearance where you have to read back the whole thing, he should have just responded "Roger, 736" especially it's a request for reportting.

  • @jkiang the requirement to READ BACK all instructions was brought about after a number of accidents where the pilots simply acknowledged instructions but had misunderstood them.

    Poor judgement, fatigue, poor radio communication, poor visiuals, communication, due to bad English from tower.

    Bad judgement from KLM Captain for proceeding to take off, and PanAm for missing the turn, both Captains did not read the Tower correct!

    I am Dutch and proud of KLM pilots", and maintanance, but still remember the PanAm Captain say,

    "Ohh My God!! He is Going To Kill Us All"!!!

    Fog, fatigue, bad comm., irritation, delay, bad judgement,

    etc. etc.

    Last error and call, KLM Captain, overruling Co KLM Pilot in taking off, after Co Pilot informed KLM Captain not to proceed with takeoff run!

    Rules changed and Co Pilots can overrule now!

    Tower and both planes had zero visuals on either!

    Open eyes, experience and respect made flying safer!

    Nobody confirmed runway clear. The air traffic asked Pan Am to report when runway is clear, and Pan Am answered that they will report runway clear.

    KLM did not have permission to take off but it did.

    The ATC didn't, repeat, didn't clear either plane for takeoff. ATC asked for PAN-AM to report when they cleared the runway, in order to let KTM takeoff. KTM was given its ATC clearance (which is NOT permission to take off) only, but KTM either misinterpreted it as takeoff clearance, or didn't care.

    * The tower said ''runway clear'', so they thought they could go.

    * The tower gives false information about which C they should take, so completing their taxing takes mutch longer.

    ATC should have identified the taxiway in question (Charlie-4) instead of simply saying "The third taxiway". Had I'd been PIC of Pan-Am I would have asked for specific instructions ("Taxiway Charlie 3 or Charlie 4?")

  • When I took my ground school and private pilot flight training we were drilled over and over again about not pushing the weather limits. So many fatal crash statistics were from pilot error doing just that as pilots would not use good judgement for reasons just like in this video to get home ASAP. The KLM pilot though being the star pilot of their line took off without tower controll authrozation resulting in so many needless deaths including the 55 children on board of his craft. So tragic.
