OK, let me explain this in a concise way:

There's a character from the Touhou danmaku series called Sakuya. In her first appearance (in the sixth game), she had a very flat chest, but later on, art coming from the game Immaterial and Missing Power (I think), showed her breasts being significantly larger. This gave rise to the rumor that Sakuya uses pads to make her breasts larger.

Also, I get that joke.
