This song is in my opinion about how his life has never gotten better and even the love of his life had betrayed him. When the lyrics say " here we are, fighting and trying to hide the scars" I believe he means the scars that his life had put upon him. My lady and I truly love this song and As he says

"take a breath and softly say goodbye" I believe it means she's leaving him and he feels alone in the world, and he walks the road alone because there's no one to help him anymore
