I stopped one jumper whose pilot chute was half way out of the pouch as he was about to exit, stuffed it back in, than informed him he was good to go. Actually I believe he cut free and then used is reserve chute. The plane on the other hand had to play around with the rudder and elevators to get the parachute off of his wing so that the elevators would respond correctly for landing.
He did not die, he cutaway that parachute and deployed his reserve. He was also wearing a camera so there is POV video of him bouncing around back there and then chopping.
I had a my first jump last summer, I can strongly recommend it! It really makes you feel like the king of the world when you get down on the ground! ;-)

looks like his pin just fell out... you can see the dbag fall out while the pilot chute and hackey are still in the rig. he's lucky he didn't hit the tail. always check your pins.
