havn't finished playing planescape torment but I sure how my hero wasn't stupid enough to hit on her to end up being sliced up and beaten like he just did.

The Lady of Pain is the ruler of the city of Sigil a city that is effectively a giant crossroad in the multiverse. It was a city in epic level handbook 3.0 and a very fun one at that. They never gave many (if any) details on the Lady of pain but worship of her is forbidden and she's powerful enough that no one's ever really meddled with her city. Considering it has a bunch of cocky adventurers that reach epic level, pit fiends, angels, and so forth that says a lot.
Everything you said is correct, plus Sigil goes back as far as 2nd Edition, where it was the centerpiece of the "Planescape" campaign setting. The Lady of Pain was sort of like the setting's mascot.
I knew it went back further than 3.0 just fuzzy on the details. Never played 2nd edition to my regret although I've heard it was the best of the editions at least in the area of roleplaying. And just so I don't cause a big argument, I'm sure each is good in one way or another and it depends on individual opinion, a lot of people I've met however advocated for 2nd edition.
I'd say 2E definitely went heavier into flavor text and world-building than 3E, yes. However, the rules were needlessly complicated and unintuitive, and by the end of its lifespan 2E had picked up way too much baggage. Just mentioning "THAC0" can make some gamers foam at the mouth.
One problem: that isn't the Styx Oarsman. That's the Black Sails tavern. They screwed up and used the name of one with the appearance of the other. They're pretty iconic within the setting, so that's pretty damn sloppy. 4e fails yet again.
I suspect most of the critics are of course, your usual youtube idiots, or are painfully unaware of DnD lore.
Trying to Hit on the dread Lady of Pain of sigil is probably the quickest way to either a quick annhilation, or an eternity of pain in her prison, The Maze.
That said, it was fairly amusing, and the animation was well done.

It just occurs to me that I wouldn't dare even make an illusion of the Lady of Pain while in Sigil...she's THAT scary!
The little guy goes in to pick up and the evil pain woman causes him some grief ie: a drink in the face followed by a knee to the ghoulies etc. but still he persists.
Then a big minotaur or somthing comes up sporting a huge scowl and says "scuse me sir, but is this lady bothering you?"
"What? Oh no, no. " The beaten reporter asures him. "I'm perfectly..."
"What have I told you about harassing the customers, Hagra?" Says the Minotaur. "Come on! You've been warned. Out!"
The reporter tries to protest as the newcomer bounces the Pain Lady out the door before turning his close attention back to the reporter who stands speechless.
"So sorry about that." He says chumily as he gently pats the Little guy's back. 'You'll have to excuse some of the weirdos we get in here."
"N.. Not at all... I..." He stutters as the giant bull-man playfully tickles his chin.
HAHAAH, I was not expecting it to be this brutal, but it just kept going on and on. I laughed a lot at "we're just going to be friends" and how his eye rolls off.

Yeah... in D&D the Lady of Pain is someone that is so scary the gods don't mess with her and don't set foot in Sigil because of it.
I wasn't terribly enthralled by this one. It had some moments but I think the Lady of Pain was a bad choice for this sort of comedy.
