Big Hairy Guys solution;

Step one as stated by Queenofdiy, glue sides. 30% vodka of course.Use a router(no not a wireless kind). The corners don't have to be square.

Drill a pilot hole to the depth required in the certre of the open book. Using the depth gauge on the router drill down then work the router around to the desired shape. It might help to clamp the book to a work bench. Don't use any kind of safety equipement as they are for pussys. All the sides are even,use spray glue to seal.Endof

Womans solution: Nice and clean and tidy, pretty, almost perfect, uses annoying glue and water mixture, uses an X-acto knife and takes forever to do.

Guy's solution: Doesn't look pretty but your the only one looking at it, uses a drill to cut massive holes in book and then rounds the edges with a saw, takes about 30 minutes, uses rubber cement to make pages stick, super fast.
