In times of pestilence doctors put herbs and medical plants in the beak of this mask to prevent infection from pestilence.

The horse in the village is "famine".

The whole scenario is "war".

I forgot what or who is "death".

The guy on the cross is a reference to Pestilence, one of the Four Horsemen of the Apocalypse. The other 3 Horsemen are also referenced in later parts of this level.

Hey, just so you know that bird dude with his hands strung to those two levitating tapestries was one of the four horsemen of the Apocalypse. I know since I read the comics. There are other omens that represent the other three.
