Malfunction. It could be a configuration error whoever was the engineer on site forgot to disable the yellow light. Or if it ran on a timer that requires the yellow light to flash at night, it malfunctioned. Dangerous setup on a highway to flash yellow. It's better off running normal 24 hours--just ask them to change the light more often late night.
the way ive heard: flashing red: treat as stop sign (which people in my area have done) flashing yellow: proceed slowly with caution. so, i assume it was a malfunction. it is weird though. never seen that in person
Basically, if you followed the lights, you need to to go for 0.5 seconds and stop in the intersection. Then after 0.5 seconds you can continue to creep through the intersection for 0.5 seconds. Then you need to stop for 0.5 seconds. Repeat this until your completely through the intersection.
It seems to me that the conflict monitor kicked the intersection into flash mode (flashing yellow) and there was a problem killing the red signal, which was malfunctioning. Or something like that. Just because the red is flashing at a different rate than the yellow, so one is being controlled by the flasher unit, and one is not?

We had a problem in Plaistow, NH a few years ago at an intersection on Route 125. Alternating flashing lights. The green lights were steady, but there were flashing yellow as well. I think the red lights were steady as well. Clearly malfunctioning lights. They've replaced them since then.

However, it was decided to get rid of the green and flashing yellow, and use a traffic light instead where the green light is replaced by a flashing yellow (so red, yellow, flashing yellow).
What happens when the fail safe unit malfunctions? I think that WAS the failure unit, when there was a short between two lamp circuits. It probably intended to flash red all around, but one red circuit was shorted to a yellow circuit.

I too have never seen that before. When ours break they flash amber or in less severe cases go all red then have a think about it for a while then go back to normal.

It's an error. Never seen that as a normal sequence here in Ohio, in Mass (home of unusual traffic lights) or anywhere else.

Around here the normally go to flashing red or just go off completely. never seen red-yellow-red-yellow before.

Our's do that under faults only. Mostly when they get tripped with a reset it's all reds.
