Ok, so first, as usual, we should blame Julie's son because he didn't buckle up. But I think Julie has the biggest share of the Blame Pie because she's being ultra-paranoid of the lorry. She's all like 'There's a lorry on London streets, crawling down the alleys and slamming the cars, so hide your sports, hide your MPVs and hide your vans because he's... OMFG a car!!!1!' If you can't concentrate on the road, you never will.

2) Like Cnjpe said, the strongest part of his head hit the weakest part of hers (the top of her spine). He was probably concussed slightly, and had a broken nose. I doubt he would actually realise what happened until about 10 seconds after the ad finished...
