If you're referring to the part where she spewed up black blood, I think I have more of a Canonical and Sensical answer. In MGS2, we learn that Operatives don't use their own blood in the field (at least the good guys), they use Artificial Blood so the Nanomachines can keep better track, and also create more. Her Artificial Blood was most likely Black, to symbolize the Octopi that her Village was famous for, same as her name "Octopus".
