If you listen to the sound as I direct the camera, you'll be able to tell that the rattling is coming from top of the DVR motor, not from the eccentric. The problem in this case was that the belt tension was too high, not that the eccentric was un-equalized.
Yes this will happen if the eccentric bushing is out of alignment. Loosen the screw on the eccentric bushing and adjust the bearing to the right or left till the sound goes away.
