@dougspair Probably a double load. Pistol powders are very dense and a second charge in the case while reloading is easy to miss. As most pistol powders have a very quick burn rate, a very small overcharge can lead to a excess pressure condition. Fatigue is not a common failure mode in modern firearms, SAAMI pressure standards are set low enough to prevent fatigue from setting in, however, It is occasionally seen in Aluminum alloys and other more exotic materials.

I've been involved in welding and metalworking for the last few years, and failure analysis is interesting to me, although many welders just 'run beads' and look for the beer after work. Aluminum and many uh, 'stainless' alloys are indeed plagued by work-hardening so that's why I asked about the firearms. I'd think the firearms munafacturers had the products pretty well developed, but of course you can't pevent abuse by the owners. .

The least likely explanation is manufacture defect. Hidden cracks or flaws in the frame. Second is more likely but still pretty far off, is poor maintenance. And the third and most likely cause is stupid people loading their own ammo and not knowing what the hell they are doing.

I have been hand-loading the .44Mag for years, and got my first .44mag @40years ago; a failure of this type is not a fault with the Colt Anaconda. This was caused by a hand-load...most likely a double charge. The .44 Mag case is large enough to double charge with fast powder. That's why it is always a good idea to use a powder that gives a 70% or better load density. Then if you do double charge a case, it is immediately obvious as the powder will overflow, giving you a visual warning,
