The cause of the 1918 pandemic was likely caused by the mass vaccinations that were done the year before.

There has been evidence that the mutations happen at the site of the vaccination so the best way to design a pandemic would be to have another mass vaccination program.

The only countries that were spared the 1918 deaths were those that skipped the vaccination program.
