The audio commentary on the DVD made it clear, that it can be seen either way.
It could be his mother as well as his wife/girlfriend.
I personally think it´s his wife ("darling"). Such guys are known to have a strange desire to marry the type of woman that is like their mother.
I´ve also read the book.
Some scenes were deleted from the movie. Especially where Peter Foley is shown as a kid, being abused by his mother...

That SWAT team scene reminded me of that level out of SWAT 4, the one where the two teenage girls are held in the lower basements
