I believe that the mechanism for the hatches corrodes over time and the hatches spring open and usually the hatches come off and land on the seabed. This would have happened long after the sinking.

Why would both forward and rear hatch be open. The SA ship had hedgehogg contact bombs. That according to records hit once.
