I've never dropped a call on my iPhone 4 with or without the case tbh. Was on the phone for about 5 hours total yesterday, only to get off the phone to go do something, such as get food etc. It's seriously not a problem at all to me. Of course, I don't hold my phone like a monkey either ;D.

if u hold it with ur left hand on the bottom left corner the call will drop
