No comment on the form - we all do it ever so slightly different but from a filming perspective the 2nd half of the form is completely lost.
If you can't get a camera operator then I'd suggest filming from the other side "as well" and cut the two together. Or as a worst case scenario film from an angle of 45degrees to when you come down the middle.
