You forgot to point out how when drug companies and med tech companies figured out the consumer wasn't paying (no market competition) the cost skyrocketed and turned $12 over the counter drugs into $150 by prescription. If I ever get very sick or need a surgery I just go back to Italy and check in the hospital for free and stay there as long as needed also get great service I had health insurrance and got horrible service here in the US I paid about $200 a month, a ticket to Italy only coast me between $600 to $1500 depending from the month, I only needed a surgery once in about 10 years so flying back is much more convinient for me.
what about federally mandated programs like dod tricare/champus? they are not insurance so where would they be managed as you cannot apply the same rules as insurances do. How do we make sure our active duty familys and retirees get ALL MEDICALLY NECESSARY benefits?

The problem isnt the Healthcare itself - the problem is rather the ever raising price of medicaments and therapy.
Doctors and pharma companys can raise their profits ad infinitum cause they know people will need them or suffer or die.
What we do need is a way to control profit margins of health related services and products.
