Well when people are dealing with a savage Islamic regime that kills and tortures Iranians in the most horrific ways then you are bound to get strong reactions; if you can't stand the heat then get out. Death to the Islamic Republic means that one is taking a clear stand against terrorism and savage Islamic tyranny.
