if you ask me, it seems like he's singing in the perspective of a serial killer who is near to taking his own life . murders [ usually ] have a reason for murdering people - "First time i had sex i was three" so the rape is his reason . Perhaps the person that raped him [ his mother im presuming ] is the reason he kills others, because they remind him of her . The schizophrenia is brought upon by post traumatic stress ?

As I see it, what he means by "First time I had sex, I was 3" is that his extra personality had sex with him (or him fapping: "Though you werent there, you remind me of those hands")
