Actually it wasn't the first. It was Island of Kesmai back in 1985 (for $12 per hour of gamplay =/), or if you want to be a completely technical it started with Mazewar which was played by MIT engineers through ARPnet.
UO was just the first massively popular and commercially successful game. Then companies started to see how MMOs can be a huge cash-cow, so EQ and AC and DAoC and a few others came out, then hundreds, and now thousands of them. UO just started the revolution. =P
One of my fondest memories in gaming was the brief time Richard Garriott actually played his MMO as Lord British. I only seen him once but still cool that he was doing that, helping the game to succeed like that. Other than that, Halo 3 is the only time I've seen developers playing their own game with the public openly (and then again they didn't build their company up out of the back of their car and eventually go into fucking outer space either).
