I looked into the origin of where this songs meaning came from and everything I can find thats verifiable relates to Caleb writing it about a killer from a movie or a lost vampire. HOWEVER, and I say this with an ass ton of experience, theres alot of belief by those addicted to heroin. The cold isolation, the constant feeling of ones unavoidable demise on the horizon, strangle of vein, refering to H as 'she', chasing taking its toll. Even the slow trancish dirge of the music feels like heroin.
