In NDE research about 15% have distressing experiences whereof 1/3, 5%, are so-called hell-like experiences with hellish visions etc. But this is the last and most intense level out of three levels within the distressing life-review. Hellish experiences are considered more intense inverted reviews with hauntings etc. but the core of it, Hell, when you peel off the visions etc. is the review.

A literal physical place, no. Heaven or Hell, people who die clearly go somewhere else as we no longer find them anywhere. Today we don't think like people 100 years ago and I believe its a mistake to read the Bible literally. A literal place if you mean dimension, yes. People who have NDEs leave their bodies and enter another dimension beyond this physical world. Extra dimensions are also backed up by Quantum physics and the fact that only 4% of our universe can be accounted for as matter.
About the pain: People who have NDEs experience the dimension outside their bodies as very powerful - 100 to 1000 times stronger than sensation here on earth. This means that the Light/heaven is very blissful but it also means that distress or pain is very painful if you multiply by 100 - 1000 times. This pain resembles 'burning in a fire' if you speak metaphorically. However, its not experienced as punishment but rather separation from God or love through our actions, choices, fear or guilt.
