The chords are Cm, Eb, Bb, F. With a capo on the 3rd that makes an Am, C, G, D

About the last chord (F, or D with capo on the 3rd), I'm not sure if it's a major or a minor. So I choose to play a Dsus2 - with the capo on the 3rd, you play a D chord but you leave the the high E string open.

(might well be that you knew that already, but it can't harm to explain, can it ;))

capo on 3rd fret. 5th fret f, slide to 3rd fret something similar to fm, then f, then slide back to 5th fret "fm" then back to f again. rep ! then you play am, c, g, and d on first fret :)
