ME1: Saved the Rachni Queen, kept Wrex alive, did all the character side quests, allowed all civis on Feros live, Saved Captain Kirrahe, Kept Ash and Kaiden alive (on male and female shep), romanced Liara, saved the council, and Anderson is on council

ME2: Destroyed Base, Completed Arrival, Overlord, Lair of the Shadow Broker, did all DLC missions, did all sidequests, made Aria's men go after Ish, kept romance with Liara, saved all crew/squadmates

He dies really easily actually. All people that are holding the line have a set chance of dying and Mordin has one of the highest. In order to make sure he lives keep him out of that group, either by having him escort survivors or bring him with you to the final boss.

I loved that they included the Mass Effect End (Reprise) and incorporated it in the end of a perfect run of Mass Effect 2. The first time I got the "No One Left Behind" achievement, they showed everyone repairing the Normandy instead of me looking at coffins, and then I was like "OMFGTHEYREPLAYINGTHEORIGINALM­ASSEFFECTENDTHEMETHISISSOAWESO­MENOSTALGICMOMENTWIN!!!!!"

I theorise that the reapers are simply pawns for their creators, who live in a different galaxy. Think about it, all the slave races have names that denote their job. Keppers keep the citadel's secret safe and maintain it. Collectors collect people to turn into more reapers. Reapers harvest all galactic life to build more of themselves. That means that the reapers are just another slave for something else!

The protheans gave them the name 'Reaper.' They don't have a name for their species, therefore, nothing to imply that they're slaves to anything.
