I think Joni Mitchell is a girl, and I think she's talking about being in love and having the waves of a relationship (ups and downs), and I think the guilt mentioned is because it's sex between two women and one of them feel guilty about it. overal passion and lust exists in all relationships and this is what she misses. The passion of love.

This poem simply represents a man's strong and deep lust for his women. To me it's quite usaual. Most men are strongly attractive towards there signifigant other. What I don't agree with is the way he tries to make her feel guilty of not wanting to perform sexual intercourse when he gets an errection. That's cruel.
