It's a very emotional ceremony. I had mine just yesterday at the historic Paramount theater in Oakland (April 14th, 2010). It was a memorable ceremony, a memory to cherish all my life!
No, they assign you a date and location. If for some reason, you can't make it that day, then you can ask to be scheduled for a different ceremony. I am not sure how often they occur (once a month?)...
