I'm guessing that's a ramp that he tried to climb that he slowed but changed his angle just enough for his over-raised travelling load to shift its center of gravity forward enough to tip.

You mean apart from the fact that reach trucks aren't designed to be used outside, he didn't have a clear area, the load CoG is forward of the fork centre, the forks are raised, and the mast is forward and not back on the reach motion it isn't his fault?

As far as having a clear area, I don't know where you work but my plant is far tighter then that. I can't tell by this video that his load is not centered on his forks the load seems stable to me.

Assuming the grade was constant, it would have tilted even before he moved if he had tried to lower it. He should have put the load back when he first saw it wobble.
The rule is NEVER jump off the truck; stay with the roll cage.
Well for one he had the load too high, you should only have it that high when racking, and even then it should be extremely slow, and 2, you really shouldn't use a Reach Truck for outside, that is a job for Counter Balance, that would of been OK and wouldn't of tipped, Reaches are meant for indoor flat ground.
don't forget that with a heavy load up front, the weight on the drive wheel (which is also the only wheel with brakes and steering) gets reduced significantly, the brakes wont help you if the drive wheel isn't touching the ground, there are a few spots in the store where I work that aren't level, and the drive wheel just spins if you don't have the momentum to get you through.

In all fairness. He didn't realise the weight of the pallet. He didn't lower the forks as he didn't have time, as you see by the skid marks left by the brake has he rolled down the hill.
I say well done for jumping out when he did as staying in the cab could have caused some serious damage.

You back out slowly where you can lower the load safely then lower the load about 2 to 3 inches above the ground. Where the box doesn't skid on the pavement the rest is simple.

He might have checked the weight limit; but most do not know to check the max height weight limit.
My forklift at work is a 7 series 1.8t but its maximum capacity for working with is 1.4t.
With slippers and other attachments (jigs, carpet spikes) this get lowered much further. a 5t fork can be reduced to 2t easily.
