@ohyan2912 You should head to Northwest Arkansas to teach. I used to work in live event production, and some of the riggers around here are insane! Climbing out of the lift or standing on the middle rail and leaning waaaay out of the basket to reach something instead of lowering the lift and driving it 10 ft.
looks like the concrete sidewalk opened up due to an undergorund cavern . i seriously doubt operator error here but more like supervisor error for not checking the terrain he had his man set upon.
Where I work, they rented one of these things, the JLG 120 footer, to do some painting below roof level of our 4 story building, and on the cupolas and main tower, which is just about 115 feet up. They asked me if I wanted to be part of the crew for that job, and I said HELL NO! These things are death traps in the hands of inexperienced operators (which I am pretty sure all of us were), and even experienced operators when the unforseen happens. They used plywood under the wheels to level it! WOW
If this dude did things safe then why didn't he check the area he was using the lift in? He would have seen that cover and kept away from it. Too bad his family has to suffer because of his ignorance.
Wild how so many of these keel-overs are because of unknown ground strength or quality. Then in China, India and Russia...it could be ANYTHING that sets off a tip over. Damn dangerous business, and I salute the guys who dare to run these monsters.
ive run and Rigged a few of these big boys since ive been an ironworker, and the 135 jlg is around 45,000 lbs,, you need to know the Quality of the Ground youre gonna be using this machine on,,, ill tell you, they are really safe when youre paying attention and properly trained,, they are a Hairy ride when all scoped out to 135 feet and the wind starts blowing, but wont tip over unless youre in a hurricane or you have one of the wheels over a soft spot, which in this case, was what caused deaths
People are going on about load distribution here, has nothing to do with it really, the guy run over a man hole in a 120 foot Jlg boom, proper training would have warned this operator about being in this situation and not running over these covers, poor guy didn't stand a chance, rip
@ohyan2912 Yea we are 100% here, I worked at one potash mine job and it was anything over 4ft, 10 ft is the norm. seems a bit ridiculous. Done quite a bit of connecting and sometimes it makes it pretty difficult. structure collapse/choker breaks and yer toast.

...and to that black lady..she dont know shit...they only way it wouldve flipped over like she said, is if the counterbalance fell off, or if the bolts from the base holding the turret were to shear off...which in both wouldve been the inspectors fault.
Its a ultra boom (1350 ajp from what it looks like) and they do have two capacities< 1000 and 500 lbs.. but i does take some force to tip one over! Im pretty sure this guy didnt see the "warning" lights and alarms going off..
Wheelbase direction doesn't matter. Wheelbase extends out square. Wheel was ontop of manhole cover which broke jerking base. Machine won't go out where it don't belong because of saftys built in. It wobbles no matter which direction it's in.
It looks to me that the weight cracked the sidewalk and that caused the life to shift. Once it shifted it kept going over. It doesn't really matter if this is called a crane or a lift or a cherry picker. Somebody died and people act like experts correcting what the machine is called.

I am an industrial lift instructor in California, I don't need to point out to everyone that this isn't a crane, many have already posted what it is correctly. When you stop it there you'll all see that the side closest to the street is at a lower elevation. Now factor that in with a fully extended boom and uneven operating surface. Actually you are all wrong. This is a JLG Boomlift. (Not a Manlift) also whenever somebody sets up in one of these they should be on hard level ground. With the ability to drive at full height in these machines they need to be on a hard surface. There is approx 5 Tonne at each wheel. Also the braniac who said he wasnt wearing a harness, it wouldnt have mattered....
I work in a fucking JLG Highworker the same type..they probably disconnect the safety because when its not squarelevel it doesnt go up..Or they used it as an krane to lift things but again when its to heavy it wont go up...
JLG articulated stick boom lift. Operator Failed by moving the lift fully extended. Never drive while extended. These lifts are deceiving, though well and look safe they can easily tip if your out of level. The cover in the sidewalk was made of a lightweight concrete of some sort, strong enough to hold a light car tire. maybe.

From listening the machine broke through a cover of some sort,you never drive one of them over a cover unless you've steel plated it first,if not you don't do it,also why was no one at ground level as look out?.
I've operated mewp's for years in a vast array of places and at full height they are certainly not for the feint hearted but you never take chances or short cuts.
Just have to wait for the inquiry to see what the final result is......

We call them an EWP in Australia (elevated work platform) THe sad thing is from what Ive heard in the states you dont need any kind of ticket or licence to operate alot of gear including ewp's or even cranes so the poor blokes workin on them as good as they might be have no real training and its the poor worker who gets the blame when things go wrong. If I were caught in 1 of those without a permit or licenced operator Id get 1 heavy fine over here
I acutally was tied down in one of this with a harness and was driving and I hit a bump on the road and I almost sling shot out the basket but the harness kept me from fly out. It was not cool. One got a bruised leg and broken pinky.
@mcgenetics You are correct in saying that this is not a crane as it is a man lift & i'm sure it was the operator that was killed ! The JLG manlift should not move after the boom is extended a given number of feet. The
mobile ability should lock out if the boom is extended so many feet, determined by JLG corp.
you know i just had a little lift accident not to long ago right down the street from there Lucky the machine shifted towards the building and the god damb hole wasn't that deep, those god damb covers a a joke there made of some fiber glass material i dont get how the city of Philadelphia expects us people to work when the streets are so tiny and so little room to maneuver hopefully they finally do something about this
That cover was over a hole, and it broke, that's the cause. That could have happened to anyone who assumed the cover was as strong as the rest of street. I've used these things, it only went up 30 feet, a slight wobble at the bottome is magnified when you're at the end of that arm in the basket, scared me to death, I came right back down. I can't even imagine what a little wobble caused by uneven pavement would feel like at 125 feet. The design is not stable, every joint is loose.
Driving it with the boom extended is always a risk. Lowering the boom to move takes time and time is money. Looks like they're spending the money now. Unfortunate for that man and his family.
weight of machine cracked the "street cover" next to the curb,( you can see it from the video) once it cracked, the tire fell through it and threw the machine off balance, resulting in it's collapse. The weight of machine should have NEVER been on the cover .
It's NOT a Crane,,,It's a " JLG " hydraulic lifting manbasket,,,,turnover very easy if untrained, unprepared, unskilled person was trying to operate it, on unlevel surface or over-extend boom

Maybe he had a chocker shackled on the bottom of the basket and was raising something in the air 120 feet. Must have been heavy and turned the JLG lift completely over. Thats what happend. Another dosh bag. Theres no other way that could have happened.

We call them an EWP in Australia (elevated work platform) THe sad thing is from what Ive heard in the states you dont need any kind of ticket or licence to operate alot of gear including ewp's or even cranes so the poor blokes workin on them as good as they might be have no real training and its the poor worker who gets the blame when things go wrong. If I were caught in 1 of those without a permit or licenced operator Id get 1 heavy fine over here

I acutally was tied down in one of this with a harness and was driving and I hit a bump on the road and I almost sling shot out the basket but the harness kept me from fly out. It was not cool. One got a bruised leg and broken pinky.

mobile ability should lock out if the boom is extended so many feet, determined by JLG corp.

you know i just had a little lift accident not to long ago right down the street from there Lucky the machine shifted towards the building and the god damb hole wasn't that deep, those god damb covers a a joke there made of some fiber glass material i dont get how the city of Philadelphia expects us people to work when the streets are so tiny and so little room to maneuver hopefully they finally do something about this

That cover was over a hole, and it broke, that's the cause. That could have happened to anyone who assumed the cover was as strong as the rest of street. I've used these things, it only went up 30 feet, a slight wobble at the bottome is magnified when you're at the end of that arm in the basket, scared me to death, I came right back down. I can't even imagine what a little wobble caused by uneven pavement would feel like at 125 feet. The design is not stable, every joint is loose.

Driving it with the boom extended is always a risk. Lowering the boom to move takes time and time is money. Looks like they're spending the money now. Unfortunate for that man and his family.

weight of machine cracked the "street cover" next to the curb,( you can see it from the video) once it cracked, the tire fell through it and threw the machine off balance, resulting in it's collapse. The weight of machine should have NEVER been on the cover .

It's NOT a Crane,,,It's a " JLG " hydraulic lifting manbasket,,,,turnover very easy if untrained, unprepared, unskilled person was trying to operate it, on unlevel surface or over-extend boom

i bet they cut corners and didn't bother to take classes on how to use the thing. either that or the inspector didn't make sure the crane was safe to use. or the idiots over loaded the thing thinking they could get done twice as fast DX.
