I have one, except mine doesn't say local alarm. It needed to be fixed, as someone pulled it in a weird way and had the pull handle all crooked. I finally got mine working.
I've seen some of these in some museums in Chicago - not on display (ha,ha), but part of their systems. One is the Museum of Science and Industry, the other is the Art Institute of Chicago.
That's an interesting pull. Those are some big problems that you listed. I would think most places would be replacing them soon if they haven't already.
My school has a similar pull station.

That's not a very good design at all. It's not even tamper proof.
