It's a combination of 2 things

1. The eyes (they clearly look evil, but by themselves they aren't too scary imo. Just showing she's not herself)

2. The smile (combined with the above however and you have something that may follow you into your sleep).

Some smiles show joy and happiness. Some smiles just show a grin.

This kinda makes sense, seeing as how Adeleine was a boss in a previous Kirby game. This boss fight was fun too, but Adeleine herself was beaten in one hit. I still wonder what all those things she painted during the fight were. One of them looked like a censored blur.
