Supine? Isn't it called side-lying position? Supine is when the patient is at his/her back. But now, at least I know how CSF looks like. And ouch, I'm not sure if I have to do this as a nurse but I know I need to know this because I will be with the doctor as a nurse, taking care of my patient.
Had it done on 9/30/10. Could not be done with this procedure, the first Dr. had a hard time. I had to be taken to X Ray to get it done. Hurt like hell !!!! Still have a headache 10/02/10. Hope my kids don't have to go thru this...

I am a doctor and am used to doing aprox. 5-10 LPs a week. If your doctor is doing it right it shouldn`t hurt much more than a neddle puncture to draw blood (there are more pain nerve roots in your arm than in your back).
Most of my patients actually fell relieved after the LP and say it didn`t hurt as much as they thought it would. Some even fall asleep during the procedure.
I have a question about the application of the lidocaine. Do you place it in a ring around the site of entry or do you actually advance it through the layers along the track that your needle is going to go? (ie. like pierce thru the skin, thru the ligaments, and all the way to just before the dura)? Thanks in advance doc if you can answer this.
I know what that feels like. I have had about 50 plus. Thanks for the video I have been on the other side. I never have seen on done. I only know what it feels like having one done!! My opening pressure has never been that low.
Mine was preformed sitting up hunched over the meal tray cart thing they have in all the rooms with a pillow. I've had 5 LP's so far and the hunching over is much less painful and easier to reach the CSF.

i would say this would hurt pretty bad, unlike the people who were saying it didnt hurt for shit. my reason being that i have a natural high tolerance to local anesthetics.
It HURTS. Especially when they hit bone. It's a blind procedure essentially so it's scary as well. You can get paralyzed, nerve damage or anything. You know it's risky when you must sign a release form to do it.
true, there is some risk with this, but if it even occurred, nerve damage would more than likely be minimal, because I'm pretty sure they know where all the major nerves are. More than likely, the biggest slip up concerning nerve damage would be slight, localized paresthesia.
I have had 11 Lumbar Punctures (relating to BIH) and each one hurt like hell to me, even with Midazolam and Nitrous oxide as well as the local anaesthetic.
BUT it all depends on the person, anaesthesias given and the person performing the Lumbar Puncture. There was this one person who did it twice on me and she made less painful then the other 9 I had.

A question for the creators of the video. What's the margin of error in this procedure? Would you be able to tell if you've gone deeper than you should have. The spinal cord is not that far away from the tip of the needle :P.
Actually at that level (L4-L5) the chord is in a loosely arranged mess called the cauda equina (horses tail) and will mostly move out of the way if encountered by the needle, also there is a bulging of the meningeal sack giving one more space (more room for error). The actual spinal "chord" ends at around L1. Hope this helps. (Medical Student)

They do, and they did it here too. That's what Lidocaine is used for, it's a local anesthetic.
Local anesthetics are prefered over generalized more often now, especially for minor procedures like this one. They are just as effective, have less side effects, and there is no need for the patient to 'come out of it' at the end of the procedure.
I had an epidural steroid injection and a facet block done with the lidocaine and it helped a lot. I even have lidocaine pain patches for extreme back pain and it helps. In terms of pain, dental procedures are much worse.
i had one yesterday with out local anaesthetic, and it didnt hurt that bad, so stop crying like a little girl. it was not pleasent but the pain level is the same as what you experience while getting a blood sample taken, so calling it extremly painfull and horrible is bull
I had this done a few times when I was a child! I am happy I really don't remember it. However, I will dread it if I have to have it again! I have horrible anxiety and may need one so I will be freaking out!
Just had one done about 17hrs ago. It wasnt too bad, they used a local so the most pain I felt was from that being injected. I just wish I could have seen it, a mirror or something would have been nice
Had this procedure done yesterday, very painful during the procedure because of a lack of local anaesthetic. Had to wait during the procedure while they got some more. Also took the doctor at least four attempts to get to the spinal fluid, then it stopped dripping so she had to remove it and do it again. Since then my back has ached non stop and the pain around the area of insertion is unbearable if I make contact with anything e.g. a bed or chair. It has me quite worried.
I've had it done 4x, as well as a spinal blood patch from too much fluid leaking out (the tear in my dura never closed, so they took blood from my arm and injected into the previous lumbar puncture site so it would form a clot and seal the hole).
Didn't hurt too bad, considering the pain of my meningitis was WAY worse!
I have to get one every 9 months...Not sure about you Princess, but everytime I go into the hospital to get my procdure. I ALLWAYS GET STUPID RESIDENTS! And they damn act like the are drilling for oil. When they do get it in...I wanna yell PAY DIRT.....WE GOT OIL!!! HAHA

normally, csf is reabsorbed as fast as it is formed by the choroid plexuses. its rate is 20 mL/hr.

This patient is very thin. It is much more difficult on a person that has a lot of fat.

Tips if you are going to have one of these:
1. do not drink any carbonated beverages before hand. During your LP the legs are pressed against the stomach.
2. pee before the LP.
3. lay on your back as much as possible for the entire rest of the day. Spend the next 48 hours resting.

Man, I had one of these not too long ago and I always wondered what it looked like from the other side. The person who did mine was not very good and didn't have much experience though because it hurt like hell and he misinformed me of the procedure, idiot.

Excellent video - I just started my ICU rotation as an intern this week, and this video is a great visual supplement that will surely help me in my first few LP's over the next month. The music is... interesting, but the video is fantastic nonetheless.
