  • when people become complacent and arrorgant tragedies come. Sadly this happened twice with the O-rings on Challenger and the panels on Columbia

  • But they did have the military take a look at the BOTTOM of the ship with a KH spy sat. Just to be sure all was well. Too bad they did not care that much on Columbias last mission :(
