Yeah. Repulsion and Carcass are both Grindcore, but they both had elements later used by Death metal bands and they both had a gigantic influence on the early scene. So I would consider them both as some kind of proto-Death metal bands.
Agreed. Beginning from the "Heartwork" era, Carcass really had many melodic elements in their music. But the real originators of Melodic Death metal are, in my opinion, were bands like Dismember, Sentenced, Amorphis, Desultory, etc.
Even so, Carcass had a great influence on many later melo death bands too.

I haven't heard Massacre demos, I must admit, but I think you're probably right. Usually demos and the second full-length studio album are the best, bands usually produce.
