You are breathing too shallow, your shoulders should not raise even the slightest if you are breathing correctly, and you shouldn't have to breathe so often. Also, be careful with the sliding that you do to get the notes, sing all the way through the note, don't just cut it off. You have a lovely voice, just some techniques need work and it will make it so much better!
