It's called Playground_Revolution, and it can be played in Portal: First Slice, just rename the file to testchmb_a_00 and put it in:

C:\Program Files\Steam\steamapps\(your steam username)\portal\portal\maps

Start up Portal normally, and select the first test chamber. Just google "Playground_Revolution" and you should be able to find it.
