If he had been holding it properly with his thumbs on the other side, the weight wouldn't have free fallen from the sky slamming into his chest. How about maybe blaming the spotter who failed to use an alternated grip? One palm up and one palm down may have been able to help with not allowing the bar to drop that easily. No communication was used at all.

It's not even a natural or functional movement anyway in life, not useful for heavy works, gymnastic, climbing, other sports. Why has it become so important as benchmark of strength? You can press with safer dumbells, develop chest through variations of dumbell flies. If you insist on wanting that type of steady motion, do deep weighted push ups instead, where you have the safer floor instead than the bar
