you can actually do it much quicker if you alter the scale by hand and click on image to show colour variation as you adjust. Still a very good video and you must have a very steady hand to use the lasso tool so accuratly. I would have jused magic wand select inverse then taken out what I didnt need with eraser before copying across

Then you need to think of the position of the head, the position of the neck.

You also need to get the lighting perfect, as it quickly looks fake if it's not. Probably the hardest part.

And you need to get a body that fits the shape of the body of that particular girl.

But i've done this thing and indeed - its hard to find a body that fits the neck and the body. Lucky i know a lot of pornstars :D The hardest part is finding the right body. But some turn out awesome :3
