They could be using a simple lighting technology during the act to prevent cameras from capturing anything. WWE are experts at lighting, pyros, and all sorts of stunts.

Because I know they tried the second with the Ultimate Warrior and Davey Boy Smith almost killed himself tripping over it, so they nixed the idea forever. It also couldn't be the first because under the ring is usually the solid concrete floor of an arena.

Maybe he comes from under the ramp. We've seen a car come up from under the ramp before, and we've also seen that the panelling is at least mostly modular. As for getting out... I really have no idea. Maybe he just does that old slight-of-hand trick where he runs away beside the ramp (in that little space between it and the barricade), and when the lights go up everyone's eyes are on the ring.
