I don't get why the uploader and the majority of the commenters are blaming the cameraman. There is more than one camera and camera operator in the room and they each have their own assigned shot they are responsible for. In this case, all this cameraman had to do was keep focus on the woman. As stated by carpenoctemcarpediem, it is the role of the technical director to switch feeds. I'll add that he, in turn, is under a director who runs the whole show. Blame one of them for the error.

That was all on the director. When I worked at a tv station we had two cameras and usually 2 people manning (I was usually 1) them and 1 person giving the hand singles to the anchors One is usually pointed at each anchor or one is filming the one who's talking and the other is setting up for another shot (like of the two of them or for the weather segment). I need to find the old VHS tapes I have of some of the funny stuff that happened. Some of it was hilarious like green screen mishaps.
