Cumulative -0.5 Minor Poison effect over 1d3 days starts on 1st day; Stage-I Nausea, Stage-II Faintness, Stage-III Fever (Fort Save per/day 1/2, round-off).

Hatching Lesions open on 3rd /or/ 4th day 1d6hrs after fever breaks & penelties depart - larvae removed by skill Profession:Chirurgeon /not/ a First-Aid/Heal use.
@ModernDnD Also, my reason for them receiving those bugs would be maybe a Vermin Lord, or some type of Rot Harbinger or something of the like made them manifest there. After all, Vermin Lord get this lovely ability to turn Maggots to Flies (Maybe the flies are very hostile and burrow to feed/lay their eggs), and well the Rot Harbinger is well....rotting, so there's bound to be insects lurking in him :P

Also, the reason I wouldn't allow them to remove the bugs completely is because they would burrow down into the player's flesh and then begin to release their toxins (if they were toxic, but I mean, c'mon, you're the DM, you gotta make it interesting/challenging). which would cause the debuff. Now, if the players were smart enough, I'd allow them to cut them out of their body and lose a little bit of HP.
About the bug idea, what are some reason why it would happen to the PCs? And what would be some reasons to keep the player from being able to remove the bugs completely? (Also: What did you think of Madness are Gardmore Abbey? Is it a fun adventure worth purchasing?)
@ModernDnD There are many reasons, if not for just bad luck. It could be the business "dirty little secret", and could be the reason the locals dont sleep there. Also I think Girhin gave a couple examples too. Just the mystery of the whole situation is enough to leave a lasting memory in the PCs minds :) Some things are better left unknown
The obese dragon idea sounds really fun. He could even have kobold servants (perhaps a couple of giant kobolds as well) that bring him his food and are too afraid to strike back against the dragon.
as always, great video! The first idea reminds me of something similar I did to the players a while back. I had this one player who would loot every monster and argue if there wasn't gold on it. He would say, "yeah right, c'mon, a goblin would have at least a few gold". So the next dungeon, the party entered a room with dead bodies and of course captain greed was in looting heaven. For every corpse he looted, a rot grub swarm erupted from the corpse 3 rounds later.

as always, great video! The first idea reminds me of something similar I did to the players a while back. I had this one player who would loot every monster and argue if there wasn't gold on it. He would say, "yeah right, c'mon, a goblin would have at least a few gold". So the next dungeon, the party entered a room with dead bodies and of course captain greed was in looting heaven. For every corpse he looted, a rot grub swarm erupted from the corpse 3 rounds later.
