It skips the 4th level, the building is only 12 floors tall.

I think this could either be the older version of the Otis Fixture or made by other elevator fixture company.

It sounds like an older Otis fixture. These elevators were modernized.

The interior have new fixture by Otis while the hall fixture are possibly by EPCO. I checked out there. Otis chime aren't sound like that.
